Prevention & Diversion
Pillars’ Prevention and Diversion programs help adults and families avoid shelter stays and maintain housing. Eligible households are typically staying with friends or family, residing in a hotel, or are at risk of losing their existing housing. Candidates are pulled from the Coordinated Entry List, which is maintained by the City of Appleton. To get added to the list click HERE or call 920-427-2769.
All households will complete an intake assessment. While awaiting case management, those seeking this service will be provided with information, referrals, and resources to self-resolve during the wait.
An initial meeting with a case manager will be held to assess the household’s situation and formally enter them into the program. The household will then collaborate with their case manager for support and resources, and to work through barriers. Together they will set goals to ensure ongoing stability. Goals will be reviewed at 30, 60, and 90 days.
Households who are staying on the street should refer to the Street Outreach Team.
Street Outreach
Pillars’ Street Outreach connects unsheltered individuals to shelter and community resources. Many of these individuals are struggling with mental illness, substance abuse, and/or trauma. Street Outreach Case Managers establish trust with their clients and work on stepping stones toward the bigger goal of getting housed.
Supportive Housing
Pillars Supportive Housing provides a critical “next step” out of shelter and is designed to meet the unique needs of specific populations, including families fleeing domestic violence, veterans in need of a home, and individuals experiencing chronic homelessness. Supportive Housing pairs affordable housing along with direct support and case management. Programs include Tenant-Based Rental Assistance, It Takes a Village, Rapid Rehousing, and Single Room Occupancy. Candidates for these programs are pulled from the Coordinated Entry List. Pillars receives funding from the U.S. Department of Housing and Urban Development and/or a federal, state or local public or private entity to provide services for individuals and families who are homeless or at risk of becoming homeless. In order to receive this funding, this agency must participate in the Wisconsin Homeless Management Information System (HMIS), which is administered by the Institute for Community Alliances
